PHC Extended Telehealth Services Program
Monday-Friday: 6:00 PM to 10:00 PM
Saturday-Sunday: 9:00 AM to 6:00 PM
Follow these procedures to setup an after-hours telehealth visit:
- Dial 305-888-7505 (If you reach voicemail, please leave a message with your name and phone number and you will be contacted shortly thereafter)
- You will be asked to provide patient information to the Extended Telehealth Services Administrator.
- The Extended Telehealth Services administrator will contact the employee and setup a virtual visit. (Click Here for patient instructions)
- A Physicians Health Center provider will call the patient and video conference with the employee through a secure video chat.
- The following business day, a copy of the DWC-25 for the telehealth visit will be emailed to the employer contact on record.
